How To Refer Your Patient for ABA Therapy

Step 1: Complete the Referral Form

Step 2: Upload Supporting Documentation

To begin the intake process, please include:

  • Diagnostic evaluation
  • Insurance card (front and back)
  • Referral form (if required by insurance)

You may upload these documents directly through our online form or fax them to (605) 271-3956.

Step 3: We Take Care of the Rest

Once your referral is received, our dedicated intake team will contact the family within one business day. From there, we walk the family through each step with clarity and personalized support.
